Disclosure of Investigation Related to Misconduct and Authorization For Release of Employment Records
Before an offer of employment with tenure will be finalized, candidates must disclose any pending investigations or previous findings of sexual or professional misconduct at their current or prior places of employment. The relevance of information disclosed or ascertained from this process will be evaluated by Indiana University on a case-by-case basis. Prior findings of misconduct or current investigations of misconduct do not automatically disqualify you from consideration and all information provided by you and your current or previous employer(s) will be considered by the relevant University offices.
Please fill out the disclosure information below and attach the signed authorization to authorize your current and previous employers to release their records of any findings or pending investigations. The disclosure information you provide below is for Indiana University’s internal records and will not be shared with your current or former employers. Current or former employers will receive a copy of the authorization form that you attach below.
